Reels use the same link as any other video
You do not need a separate Reels mode inside the Threads app to get a usable URL. Open the Reel, tap Share, and copy the link. Paste it here. If the post is public, the preview shows the clip and Download saves an MP4.
Vertical video is still just video. The file is not cropped into a square and it is not turned into a GIF unless the post itself is a GIF. Sound, when the upload has sound, stays in the MP4. You can also save only the soundtrack as M4A from the audio button.
When a Reel will not load
Private accounts, deleted Reels, and links that point at a profile instead of one post return no file. A Reel you can see only because you are logged in is not a public file. Test the link in a logged-out window. If it asks you to join Threads before you can watch, this downloader cannot fetch it either.

For saving, no. Both are a public post URL and an MP4 when a video file is available.
Do Reels download with sound?
The MP4 includes the audio that was uploaded with the clip. You can also extract that audio as M4A.
Can I save a Reel on iPhone?
Yes. Use Safari, then move the file from Files to Photos with Save Video if you want it in the camera roll.
